I am so excited to get back to school and see all the students at Mitchell! This year is sure to be different, but different forces us to grow, and growth is good.

For students who require medication during the school day, medication drop off will be by appointment only.
Please refer to the district's Medication Administration in the School Setting page to printout the required forms. You must have all of the following items in order for any medications to be accepted:
*Parent Authorization for Medication Administration in School Form
*Physician’s order form - Can use this school template or the one that the Doctor provides. We do not require both.
***In addition, students with life threatening food allergies also must have:
*Emergency Health Care Plan (Allergy Action Plan)
*3 Wallet size pictures of your child to be placed on the emergency Health Care Plan
Once you have printed and competed all the required forms please call to make an appointment for medication drop off. 1781-455-0466 x54042
Please bring your student's medication and completed required medication forms to your appointment. I will meet you outside in the circle in front of the school building. All prescription and over-the-counter medications must be in the original pharmacy labeled container with child's name and birth date clearly visible. Please be sure to check medication expiration dates prior to drop off.
